NEW MOTOR CLAIMS
OWN DAMAGE CLAIMS
The report may be made through telephone, email, or a visit to our office. When reporting, it is necessary to quote the vehicle registration number, name and address of the policyholder, date of accident, circumstances surrounding the accident, extent of damage, and persons injured.
Current location of the vehicle, and the policyholder’s preferred garage (from our panel of garages)
We shall appoint an assessor to establish the extent of damage to the vehicle, and authorize repairs on our behalf.
We require the following documents, inorder to process the claim: duly completed Motor Accident Claim Form, copy of the driver’s licence, and Police Abstract report Form. Any correspondence received from the other party (if accident involved another vehicle) should be forwarded to us unanswered.
On satisfactory completion of repairs, we release the vehicle to the insured, upon payment of policy excess. The insured should sign a satisfaction note before taking p[possession of the vehicle from the garage.
The insured and/or driver should not under any circumstances admit liability for the accident.
TOTAL LOSS
If it is uneconomical to repair the vehicle, it is treated as a constructive total loss.
In this case, we pay the insured the pre-accident value of the vehicle (or sum insured whichever is lower), less the policy excess.
Upon payment of the claim, the vehicle salvage becomes our property.
THEFT CLAIMS
Should be reported to us within 48 hrs of the occurrence.
The basic documents needed are a complete motor theft claim form and a Theft Abstract report from the Police
Amount payable is based on pre-theft value of the vehicle (or sum insured which ever is lower).
WINDSCREEN CLAIMS
Report claim to us, by completing a Windscreen claim form
Take vehicle to any of the branches of Hebatulla Brothers Limited, and the windscreen will be replaced at no cost (subject to limit of windscreen cover).
RADIO CASSETTE CLAIMS
Report claim to us, by completing a radio cassette theft claim form
Attach replacement receipt and Police Abstract report form
The claim will be settled in full (subject to limit of cover under radio cassette extension).

Partial Maturities
- These are processed on the due date automatically on all policies whose premiums are up to date
- For those policies with loan balances, the partial maturity amount is utilized to repay loan and any balance is paid to the client

- Original Burial permit
- ID copy or passport copy.
- Client's notification letter(signed and stamped if not on letter head)
- Payable in 48 hours.
GROUPLIFE ASSURED
- Claim form duly signed and stamped
- Original death certificate for our certification and return.
- ID Copy or Birth Certificate or passport copy.
- Client's notification letter(signed and stamped if not on letter head)
- Police abstract if death is accidental.
- Marriage certificate/sworn affidavit incase the names differ.
- Payable in 7 working days.
Group creditor/Mortgage protection cover
- Claim form duly filled, signed and stamped
- Original death certificate for our certification and return.
- ID Copy or Birth Certificate or Passport copy.
- Client's notification letter(signed and stamped if not on letter head)
- Police abstract if death is accidental.
- Marriage certificate/sworn affidavit incase the names differ.
- Relevant loan ledger card.
- Payable in 7 workings days.
MIPPP
- Policy holder's notification letter
- ID copy
- Original Policy document.
- Proof of death if deceased.
- Relevant pay slip copies IF ERRONEOUS DEDUCTIONS.
- Payable in 7 working days.
DEPOSIT ADMINISTRATION(Pensions & Provident fund)
- Claim form duly filled, signed and stamped
- ID Copy or Birth Certificate or passport copy.
- Client's notification letter(signed and stamped if not on letter head)
- Proof of death if deceased.
- Last physician report/Doctor’s letter if member retires on ill health.
- Marriage certificate/sworn affidavit incase the names differ.
- Payable in 7 working days.
GAIN PLUS
In the event of an annuitant dying within the guarantee period, the beneficiary shall furnish the insurance co. with:-
- Notification letter on the death
- Death certificate
- Copy of ID for nominated beneficiary.
